We are a family friendly, general and cosmetic dental practice that is built around the patient experience while maintaining our focus on oral health and patient education. Our goal for your first visit is that you experience comfort and ackowledgement of your personal needs.
What should you expect at your first appointment? Your first appointment will consist of a comprehensive dental evaluation. We will take the necessary time to diagnose and evaluate your dental concerns and create a treatment plan which optimizes your oral health. Based on the evaluation, x-rays may be taken. Your dentist will review your exam and x-rays with you as well as discuss how your dental needs can be met.
We are here to listen! If you are visiting for the first time with an emergency or dental pain, you will be treated as an emergency patient to satisfy your immediate needs first.
What should you bring to your first visit? You will need a photo ID, insurance card, and a list of any medications you are taking. Once you arrive, you will be asked to complete a patient registration, medical history, record release, and HIPPA form. If you prefer to fill out these forms before your visit, click the tab below.
Insurance? Let us help you navigate the confusing world of dental insurance. As a provider, we work with most major plans. An additional part of our services includes coordinating with your insurance company to find out how to maximize your benefits to best cover your treatment as well as file claims on your behalf.
Cancellation Policy. Wellesley Family Dental strives to provide the highest quality dental care possible while providing treatment times that are convenient for the patients of the practice. To support the mission of the practice, we ask that patients provide the office with a 48 hour notice prior to rescheduling or canceling an appointment. If you are unable to keep your appointment, please understand that there will be a charge of $50 per appointed hour. Our office reserves the right to deny future appointments to patients who repeatedly cancel or reschedule without 48-hours advanced notice.
